I hereby give my permission to Creative Spanish to use my child’s image for promotional purposes, such as Creative Spanish materials or written promotions, including but not limited to the Creative Spanish website. I can retract my permission in writing at any time.
Deposits/Payments
I understand that deposits and payments for Creative Spanish Summer Camp are non-refundable in the event of absence or withdrawal of any students for any reason whatsoever, regardless of voluntary or dismissal from the program if the child conduct or behavior is deemed unacceptable.
Minimum deposit of 50% is due with the completed form after it is approved that space is available. Full balance is due before the starting requested week.
